1. Overview and Commitment to Privacy
Quintana Medical Services Inc. ("Quintana Medical Services", "we", "our", or "the Practice"), led clinically by Onel Quintana Reinoso, APRN, is dedicated to upholding the highest standards of confidentiality, patient trust, and regulatory compliance.
This Privacy Policy applies to our website (quintanamedicalservices.com), downloadable educational clinical guides, booking inquiries, and administrative communications. It describes how we collect, protect, use, and respect your personal information in full accordance with applicable Florida laws and United States federal standards.
2. Information We Collect on This Website
We collect information only when you knowingly and voluntarily provide it to us through our website interactions:
- Contact and Scheduling Inquiries: If you call, email or message us on WhatsApp: your name, contact details and the scheduling information you share with us.
- Educational Guide Downloads (Lead Magnet): When you request the guide: your name, email address, optional mobile number, the guide language, your email and text-message choices (with the exact text you accepted) and the date, time and page of the request. This information is sent to a Google Apps Script service, stored in a Google Sheets spreadsheet in our Google Workspace account, and Google sends us an email notification with the same details.
- Technical & Log Data: Our website is hosted by Cloudflare, which processes technical data such as your IP address, browser type and request logs to deliver and protect the site, and may run a bot check (Cloudflare Turnstile) when you send a form. The home and contact pages show a map image hosted on our own website (map data © OpenStreetMap contributors); no data is sent to third parties when it loads. If you tap it for directions to the clinic, the map app you choose (Apple Maps, Google Maps or Waze) opens under its own policies. We do not use analytics or advertising trackers.
Our public website forms are intended solely for general inquiries and educational materials. Please do not submit medical histories, symptoms, lab reports, or sensitive medical records through this website, by email or by WhatsApp. Clinical information is handled during your consultation in Suite 310, and your medical records are confidential under section 456.057, Florida Statutes.
3. Notice of Privacy Practices (HIPAA) & Clinical Records
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
When you establish a formal clinical relationship with Quintana Medical Services as a patient under the care of Onel Quintana Reinoso, APRN, your Protected Health Information (PHI) is rigorously safeguarded pursuant to the Health Insurance Portability and Accountability Act of 1996 (HIPAA, 45 CFR Part 160 and Part 164) and Florida Statute Section 456.057.
How Your Clinical Information May Be Used:
- For Treatment: Providing, coordinating, and managing your medical care, diagnostics, lab evaluations, and preventive clinical protocols.
- For Health Care Operations: Quality assessment, clinical administration, protocol compliance, and continuous improvement of clinical services.
- As Required by Law: Complying with mandatory public health reporting, court orders, or state regulatory oversight as prescribed by Florida statutes.
We never use or disclose your clinical records for commercial marketing purposes or sell your medical records under any circumstance without your explicit, written authorization.
4. How We Use Website and Marketing Information
Information gathered through our digital channels is utilized exclusively for legitimate practice purposes:
- Fulfilling requests for educational clinical materials, including the Longevity & Key Biomarkers Guide.
- Responding directly to appointment requests, questions about private pay structures, or scheduling availability.
- Transmitting periodic educational wellness updates and clinical newsletters solely to subscribers with explicit opt-in consent.
- Maintaining the security, technical integrity and availability of our website.
5. Email, Text Messages and WhatsApp
We send clinic news and educational content by email only to people who tick the optional box on our website or ask us in writing. Every email identifies Quintana Medical Services as the sender, includes our physical address (7392 NW 35 Terrace, Suite 310, Miami, FL 33122) and a way to unsubscribe. You can unsubscribe at any time with the link in the email or by writing to info@quintanamedicalservices.com, and we will honor your request within 10 business days, as the CAN-SPAM Act requires.
Text messages and WhatsApp
We send appointment reminders and clinic news by text message (SMS) and WhatsApp only to people who opt in with the checkbox on our website or in writing at the clinic. That consent covers messages sent with automated technology, is not a condition of any purchase or service, and we keep a record of the exact text you accepted and when. Message frequency varies; message and data rates may apply.
You can stop these messages at any time by replying STOP (or HELP for help), by calling (786) 819-4066, by writing to info@quintanamedicalservices.com or by any other reasonable means, and we will honor your request within 10 business days.
7. Sharing, Security and Retention
We do not sell or rent your personal information. We share it only with the service providers that help us run this website and our communications (Cloudflare and Google), when you ask us to, or when the law requires it.
We use reasonable safeguards to protect your information, and only authorized staff can access the spreadsheet where guide requests are stored. We keep guide requests and contact inquiries for as long as we need them to send you what you asked for and, if you opted in, our communications; you can ask us to delete them at any time. If a security breach affects your personal information, we will notify you as required by section 501.171, Florida Statutes. This website is not directed to children under 13, and we do not knowingly collect their information. If we change this policy, we will post the new version here with a new effective date.
