1. Notice of autonomous practice (section 464.0123(7), Florida Statutes)
Onel Quintana Reinoso, APRN, FNP-C, is an Advanced Practice Registered Nurse licensed in Florida (license APRN 9429858) and registered with the Florida Board of Nursing for autonomous practice. He is not a physician and is not licensed to practice medicine.
Florida law limits autonomous practice to primary care (family medicine, general pediatrics and general internal medicine). If you need care outside primary care, you will be referred to an appropriate provider.

You will also receive this notice in writing before or during your first visit, as the law requires. Ask us any question about it at any time.
2. Your rights as a patient (Florida Patient's Bill of Rights and Responsibilities, section 381.026)
Florida law gives every patient of a health care provider the following rights. This is a summary; the full text is available at leg.state.fl.us.
- To be treated with courtesy and respect, with appreciation of your individual dignity and protection of your need for privacy.
- To a prompt and reasonable response to your questions and requests.
- To know who is providing your care and who is responsible for it, what support services are available (including an interpreter if you do not speak English) and what rules apply to your conduct as a patient.
- To receive information about your diagnosis, planned course of treatment, alternatives, risks and prognosis.
- To refuse any treatment, except as otherwise provided by law.
- To be given, on request, full information and counseling on the availability of known financial resources for your care.
- If you are eligible for Medicare, to know on request and before treatment whether we accept the Medicare assignment rate.
- To receive, on request and before treatment, a reasonable estimate of charges for your care.
- To receive a copy of a reasonably clear and understandable itemized bill and, on request, to have the charges explained.
- To impartial access to medical treatment, regardless of race, national origin, religion, physical handicap or source of payment.
- To treatment for any emergency medical condition that will deteriorate from failure to provide treatment.
- To know if medical treatment is for purposes of experimental research and to give your consent or refusal to participate.
- To express grievances about any violation of your rights, as stated in Florida law, through the grievance procedure of the health care provider and to the appropriate state licensing agency.
3. Your responsibilities as a patient
Florida law also asks patients to:
- Give your provider, to the best of your knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters relating to your health.
- Report unexpected changes in your condition to your provider.
- Tell your provider whether you understand the planned course of action and what is expected of you.
- Follow the treatment plan recommended by your provider.
- Keep appointments and, when you cannot, notify the provider.
- Take responsibility for your actions if you refuse treatment or do not follow the instructions of your provider.
- Make sure the financial obligations of your care are fulfilled as promptly as possible.
- Follow the rules and regulations of your provider affecting patient care and conduct.
4. Costs, estimates and itemized bills
Quintana Medical Services is a direct self-pay practice. Before you schedule, we tell you the current fees. If you are uninsured or choose not to use insurance, the federal No Surprises Act gives you the right to receive a written Good Faith Estimate of expected charges before your visit; ask us for it when scheduling and we will provide it. After your visit you can request an itemized bill and an explanation of the charges.
If you are eligible for Medicare, ask us before treatment whether we accept the Medicare assignment rate and we will tell you.
